Trevogrumab: Myostatin (GDF-8) Antibody Research Overview

Updated 4 min read

On this pageWhat Is Trevogrumab?

Trevogrumab (REGN1033) is an investigational, fully human monoclonal antibody that binds myostatin, also known as growth differentiation factor 8 (GDF-8). This reference explains the molecule class, the myostatin/GDF-8 ligand–receptor biology Trevogrumab is used to study, and its development status. It does not report study outcomes.

What Is Trevogrumab?

Trevogrumab is a fully human monoclonal antibody discovered by Regeneron Pharmaceuticals with its VelocImmune platform. Its antigen is the mature myostatin (GDF-8) growth factor, and its mode of action is ligand neutralization: by binding myostatin, Trevogrumab prevents the ligand from assembling an active receptor complex.

  • Name: Trevogrumab
  • Development code: REGN1033
  • Class: fully human monoclonal antibody (anti-GDF-8)
  • Target: myostatin / GDF-8, a TGF-β superfamily ligand
  • Developer: Regeneron Pharmaceuticals

Myostatin (GDF-8): A TGF-β Superfamily Ligand

Myostatin is encoded by the MSTN gene and belongs to the activin/GDF branch of the TGF-β superfamily, alongside activin A, activin B and its closest relative, GDF-11. Its biosynthesis follows the general superfamily pattern:

  1. Precursor synthesis: myostatin is made as a pre-pro-protein containing a signal peptide, a large N-terminal prodomain and a C-terminal growth factor domain.
  2. Furin cleavage: a proprotein convertase cleaves between the prodomain and the mature domain, but the two remain non-covalently associated as a latent complex.
  3. Activation: metalloproteases of the BMP-1/tolloid family cleave the prodomain, releasing the active, disulfide-linked mature dimer.

This multi-step activation means research samples may contain pro-myostatin, latent complex and mature ligand at the same time. Antibodies can be raised against any of these forms; Trevogrumab is described as binding the mature GDF-8 ligand.

Myostatin/GDF-8 Ligand–Receptor Biology: ActRII Signaling

Mature myostatin signals through serine/threonine kinase receptors:

  • Type II receptors: ActRIIB (ACVR2B) is the preferred high-affinity receptor; ActRIIA (ACVR2A) also binds the ligand
  • Type I receptors: ALK4 (ACVR1B) and ALK5 (TGFBR1) are recruited into the complex

Within the complex, the constitutively active type II kinase phosphorylates the GS domain of the type I receptor. The type I receptor then phosphorylates SMAD2 and SMAD3, which bind SMAD4 and move to the nucleus to regulate transcription. Inhibitory SMAD7 and non-canonical outputs such as p38 MAPK and the AKT/mTOR axis are also part of ActRII signaling studies.

Because activin A, activin B and GDF-11 use the same type II receptors, ActRII signaling is a shared node. Neutralizing a single ligand with Trevogrumab lets researchers measure the myostatin-specific share of that signal, in contrast to receptor-blocking antibodies or soluble ActRIIB ligand traps that act on many ligands at once.

Endogenous Antagonists of Myostatin

Myostatin activity is regulated by several binding proteins that serve as useful comparators to antibody neutralization:

  • The myostatin prodomain itself, which keeps the ligand latent
  • Follistatin and follistatin-like 3 (FSTL3), which also bind activins
  • GASP-1 and GASP-2 (GDF-associated serum proteins, encoded by WFIKKN2 and WFIKKN1)
  • Decorin and other extracellular matrix components reported to sequester the ligand

How Trevogrumab Is Used in Laboratory Research

Trevogrumab is used as a ligand-selective tool in ActRII/SMAD2/3 pathway studies. Common in vitro approaches include:

  • SMAD-responsive reporter assays (for example CAGA-luciferase) in cells stimulated with recombinant GDF-8, with and without antibody, to derive neutralization IC50 values
  • Phospho-SMAD2/3 immunoblotting or ELISA as a proximal readout
  • Binding kinetics by surface plasmon resonance or biolayer interferometry against GDF-8, GDF-11, activin A and activin B to define cross-reactivity
  • Receptor competition assays using ActRIIB-Fc to confirm that the antibody blocks the ligand–receptor interface

Pairing Trevogrumab with an activin A antibody such as garetosmab is a standard way to separate the myostatin and activin A contributions to ActRII signaling.

Trevogrumab Development Status

Trevogrumab is an investigational antibody in clinical development by Regeneron Pharmaceuticals. It is one of the agents in the Phase 2 COURAGE study (NCT06299098), where it was evaluated with semaglutide, alone or together with garetosmab. A summary of that study's design is available in the COURAGE study research overview. It is not an authorized product for any use.

Characterization and Storage of Trevogrumab Research Material

Monoclonal antibody research material is characterized with methods suited to large proteins: size-exclusion chromatography (SEC-HPLC) for monomer and aggregate content, SDS-PAGE or capillary electrophoresis under reducing and non-reducing conditions, intact or subunit mass spectrometry, A280 protein concentration and LAL endotoxin. Antibody solutions are generally kept refrigerated or frozen in single-use aliquots, protected from light, and not subjected to repeated freeze–thaw or vigorous agitation.
